Dear Patients,

If you are new to our office, we would like to take this opportunity to welcome you as a patient and to thank you for choosing our ophthalmology practice. It is our goal to assist you with all of your eye care needs. We wish to make your visits informative and your experience pleasant and rewarding. Please be prepared with the following items at your visit:

- A Parent or Guardian (If you are under 18)
- Medical and Vision Insurance Cards
- Previous glasses or contact Prescriptions
- Medical records from your previous physician (If you have only the contact information, please be early for your visit)
-A list of your current medications (if necessary)
-A dilation driver (optional)
I encourage you to make a list of any questions you may have. You will find we are dedicated to excellence in patient care. During your consultation we will review your medical history, perform a routine eye exam and discuss your goals for continuous eye health care.
